step 1: ditch the wet man
The hardest part of this guide is going to be not choosing waterboy in the 4th episode since you're going to need the poor fella at their weakest. This part is easy enough.
step two: hurt the wet man
The meanest part of this guide and also the easiest to do accidentally is to have him be hurt by the third dispatching section of episode 8. This will have all of his stats be at one which is what we need for this strategy. Important that this be when he's hurt as this will be the only opportunity to do so for the next step.
final step: get saved
Now you have a hurt Waterboy with all of their stats at one by the third dispatching section of episode 8, and this is where your stats come in. You send him on the first possible mission [excluding fighting coop or sonar] which for me was atomic automatons. I'm pretty sure that's the mission everyone starts with but, can't confirm. Either way first non-coop/sonar fighting mission and your success rating will be well under 15%. Now one of two things will happen: you'll get insanely lucky and hit the 4% chance of success, which, hats off to you. Or you'll get saved by blonde blazer and succeed the mission anyway. And both will net you the achievement.

Assuming this doesn't get patched out you managed to make your own luck and get an achievement exclusive to people who can phenagle their stats and saves to get a 15% chance or less on a mission. It's that simple!

I used Flambae with Supernova. His stats will be fall to lowest after fail at certain dispatch.

it would probably work with golems skill but this is just how i got it to work. i'm not saying this is the "only" way just that it works

Another solution would be to have Golem's "Found Himself" upgrade, then by the third shift reset his stat without allocating any stat points, then by the first non Sonar/Coupé mission of the third shift, send him alone, you will get the same 4% chance of success, and Blazer will come and help, thus giving you the success, either method (mine or OP's) still work as of November 29th ^^

i don't think it's "necessary" but it will make the chance of success lower and therefor is strategically preferred. feel free to prove me wrong if you find otherwise.

Ok, so I managed to do the given mission even without ditching the Waterboy in episode 4, protected the visi from being kicked from the team which resulted in team ( and importantly Waterboy ) being demoralised and having lower stats.
That lowered stats happened even before the him failing one mission and being damaged and even have lower stats.
Then waited for the last and third part of the 8th episode dispatch ( the part right after you decide for your heroes to not return to dispatch HQ after each mission ) , and sent the waterboy solo to do a mission to be saved by Blonde Blazer and auto succed where he had about 6% of success on a mission.
